Parents/Guardians:
It's that time of year when students in grades 7-12 will need to have a new physical for summer open gyms and practices. SOMC is doing FREE physicals in their local offices from May 1 to May 31 from 8a - 8p seven days a week. See the attached flyer for more details. SOMC Sports Physicals Flyer.
It is mandated by the Ohio High School Athletic Association that all students in grades 7 - 12 participating in a jr. high or high school sport have a physical completed yearly. Students will need only one physical per school year, unless the previous one has expired.
The athletic department keeps a close watch on physical expiration dates. FinalForms also helps track these dates and sends parents an email letting them know their child's physical is about to expire. (It is very important that ALL email addresses are correct in FinalForms for this information to be sent.)
It is the parent/guardian's responsibility to verify all the information in FinalForms is correct and to sign the forms. This allows the correct information to be placed on their emergency medical form which coaches are required to have at each open gym, practice and game.
The 2022-23 physical form will not be available until mid-April. It will be posted at that time. If you are unsure when your child's physical expires, please check FinalForms.
PLEASE NOTE: When the physical form becomes available, there may be some changes so please read each page carefully! It is important that all areas be completed especially those that are HIGHLIGHTED. The forms are considered incomplete, and will not be accepted, until all spaces are completely filled out and signed by both the student and the parent(s). The student will be unable to participate in any open gyms, practices or games until the completed form has been received by the athletic department.
