Green Local is partnering with the Scioto County Health Department to offer an immunization clinic at Green High School on Tuesday, May 9th from 9:30 - 11:30 AM. This immunization clinic is specifically for students who are currently in 6th or 11th grade. The goal of this opportunity is to assist families with required and/or recommended vaccines. Registration forms will be distributed to 6th and 11th grade students, are available as a part of this social media post, and upon request from your child's respective office. Parents/Guardians do not have to be present when vaccines are administered, but consent must be indicated on the bottom portion of the 1st page of the registration form. Additional details are available on the attached flyer.
